1. I have a question about my Medicaid coverage and I applied with the State, who do I contact?

You should contact the New York State of Health at 1-855-355-5777 or visit the website at nystateofhealth.ny.gov. The local Department of Social Services will not be able to provide assistance for Medicaid cases with coverage through the State. nystateofhealth.ny.gov

2. How do I add someone to my case? Who can be added to my case?

For Medicaid cases that are open with the local Department of Social Services, you can contact your worker via phone or mail to inquire about who can be added to your case and to request to add an individual to a case. In some instances, a new application may be required.

3. What changes am I required to report and in what time frame?

Any changes in income, resources, address and household composition need to be reported to the Department of Social Services within 10 days of the change. Contact your worker if there are other changes that you are unsure of whether or not you need to report.

4. I have lost my CBIC Card to obtain my benefits, how do I get a new one?

If you are in need of a new CBIC card, you will need to contact your worker at the Local Department of Social Services office.

5. How do I apply for Medicaid?

You can apply online at nystateofhealth.ny.gov or call 1-855-355-5777 to apply through New York State. You can also call 518-746-2300 for an application to be mailed to your home. nystateofhealth.ny.gov
