AXIS
The Coalition works to destigmatize addiction and support those who are suffering from the disease of addiction. Our efforts also support loved ones and those in long term recovery as well. The Coalition has held numerous community forums which have served to raise awareness and provide education regarding addiction and its broad impact.

Hope and Healing Recovery Center
Their mission is to assist and empower people, as well as family members, in and/or seeking a life of Recovery. Hope and Healing offers peer driven and delivered recovery support services in a safe and supportive environment. Recovery is a reality!

Plan of Safe Care (POSC)
When an infant has been exposed to substances during pregnancy a POSC is developed. The plan is written with the pregnant/parenting mother and her personal and professional support team.
The POSC is a written plan that can be updated and stays with the woman. She is encouraged to share the plan with her support team, throughout her pregnancy. The team helps to ensure resources are available to assist the woman, infant, and family.
The Plan of Safe Care provides an opportunity to refer mothers to important services, educate them about the health and safety of their infants, and prepare them for the challenges of caring for a substance exposed infant.

Suicide Prevention Coalition
Suicide Prevention Council is led by the Council for Prevention. The purpose of the coalition is to prevent suicide in our community by:
- Providing training in suicide prevention and intervention
- Striving to strengthen communication and coordination
- Providing resources to families, communities and schools
- Working to increase awareness and reduce the stigma of getting help

Mobile Health Care System
The Mobile Health Care System is medical care that you would get from EMTs and Paramedics in the comfort of your home. This is a collaboration with local EMS, Doctors, Public Health and more. They work together to bring Wellness Checks to people in their own homes when Home Nursing Care is not available or if someone just needs a check-in.
The goal of the programs is to identify gaps in the health care services in our communities and develop programs to fill those needs. Programs include the Community Check Program, Falls Prevention, CarFit, concussion awareness, car seat checks, and bicycle helmet fit checks.
The goal of these Wellness Checks are to reduce risk of illness and injury that can be prevented.
The Community Check Program brings specially trained EMTs into your home to collect health information that your doctor can use to better manage your overall health.

The program was originally a collaborative effort of Cambridge Valley Rescue Squad, Inc, Easton-Greenwich Rescue Squad, and Salem Rescue Squad.
Bay Ridge Rescue Squad and Granville Rescue Squad, Inc. have recently been added to the collaboration.
We have a strong and experienced Steering Committee that meets once a month to work on our programs, funding, trouble-shooting, etc. The Steering Committee includes representatives from all the participating agencies, Washington County, NY Public Health Department, local primary care practices and hospitals, our medical directors, the Alzheimer's Association, and regional health care coordination organizations.
Domestic Violence Project of Warren & Washington Counties
Services include a 24-hour hotline, emergency shelter, supportive counseling, advocacy, crime victim assistance, support groups, community education and outreach.
The Warren & Washington Counties Domestic Violence Community Coordination Council (DVCCC), formed in 1995, is a multi-agency council composed of representatives from a variety of agencies. The DVCCC seeks to bring together the people responsible for responding to domestic violence, and those who may play a role in its prevention, in an effort to further enhance their cooperation, coordination and ultimate effectiveness. The DVCCC’s goal is stopping domestic violence in our community: through seeking ways to increase safety for victims and accountability for those who batter, and by seeking to prevent domestic violence from occurring in the first place through a process of social change impacting the attitudes, beliefs, norms and behaviors that have given rise to domestic violence in our culture. Washington County Office of the Aging Page
The Washington County Office for Aging is committed to Service, Education and Advocacy to meet the needs of Washington County's older adults.
To be the entry point for information and access to a comprehensive system of consumer based services for older adults and their families, with particular attention to the socially, physically and economically isolated, such that they can retain their dignity and remain successfully independent in their homes and communities.
The New York State Office for Aging has designated Washington County Office for Aging and Disabilities Resource Center as the official area agency on aging under the Federal Older Americans Act. Funding for services is made possible through federal and state grants, as well as, the Washington County Board of Supervisors.
The Conkling Center
The Conkling Center offers senior directed programs and services for community members 55+. These include: On-The-Go Transportation Program, Ice Melt Program, and a variety of Programs touching on topics that fall within the areas of Community Awareness, Health and Safety, Legal, Educational and Inter-Generational. They collaborate with individuals and organizations in offering their programs and services to the community.
